We are seeking a skilled and compassionate Home Health Registered Nurse to provide high-quality personalized care to patients in their homes. As a Home Health RN in Hartford, CT. This position offers a competitive salary of $2480 Weekly and the chance to work with a dynamic and supportive healthcare team.
Responsibilities:
– Conduct comprehensive assessments of patients’ health conditions and develop individualized care plans
– Administer medications, treatments, and other nursing interventions as prescribed by physicians or healthcare providers
– Monitor vital signs, assess and document patient progress, and adjust care plans accordingly
– Collaborate with interdisciplinary healthcare team members to provide coordinated and effective patient care
– Educate patients and their families about disease management, self-care techniques, and medication administration
– Communicate effectively and professionally with patients, caregivers, and healthcare personnel to ensure continuity of care
– Adhere to all relevant laws, regulations, and ethical standards in nursing practice
– Establish and maintain accurate medical records, including patient assessments, care plans, and progress notes
– Participate in performance improvement initiatives to enhance the quality of care and promote positive patient outcomes
Requirements:
– Valid and unrestricted RN license in the state of Pennsylvania
– Minimum two years of recent nursing experience in a home health, hospice, or similar setting
– Knowledge of home health regulations, policies, and procedures
– Excellent clinical skills and ability to assess, plan, implement, and evaluate patient care
– Strong interpersonal and communication skills to effectively interact with patients, families, and healthcare team members
– Proficient computer skills and ability to use electronic medical record (EMR) systems
– Demonstrated ability to prioritize tasks, manage time effectively, and work independently
– Compassion, empathy, and commitment to delivering patient-centered care
– Must pass background checks and drug screening
