We are seeking an experienced and self-motivated Case Manager travel registered nurse to join our growing team of registered nursing professionals.

RN Qualifications:

• BLS, – Must have Certified Health Care Experience 
• NY RN LICENSE

Registered Nurse Duties & Responsibilities:

• Assessing patients’ physical, psychosocial, and environmental needs to formulate a comprehensive care plan
• Collaborating with healthcare team members to develop and implement care plans based on patients’ conditions, needs, and preferences
• Coordinating care across different healthcare settings, including hospitals, clinics, home care, and long-term care facilities
• Monitoring patients’ conditions, response to treatment, and compliance with medications and treatments
• Identifying and addressing potential barriers to successful care, such as financial, cultural, or language barriers
• Providing patient education and support to promote self-management and adherence to care plans
• Documenting patient care activities and outcomes in electronic health records
• Conducting ongoing quality improvement activities to optimize patient outcomes
