LPN – Long Term Care

3x12s – 36hrs – Days and Nights available

13 weeks

$40/hr – $1440 Weekly

We are currently seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at a long-term care facility in Penn Yan, NY. As an LPN specializing in long-term care, you will play a critical role in providing hands-on nursing care to our residents and ensuring their physical and emotional well-being.

Responsibilities:

 

– Deliver high-quality care to residents, following established care plans and nursing procedures.

– Administer medications, observe and document any reactions, and promptly report any changes in health status to the appropriate healthcare professionals.

– Assist residents with daily activities, such as bathing, dressing, toileting, and transferring.

– Monitor vital signs and other health-related indicators, maintaining accurate records and promptly escalating any concerns to the registered nurses or physicians.

– Collaborate with the interdisciplinary team to develop and implement individualized care plans, meeting the unique needs of each resident.

– Provide emotional support to residents and their families, fostering a positive and nurturing environment.

– Ensure the facility meets all regulatory and compliance requirements, adhering to infection control protocols and maintaining a safe and clean environment.

– Supervise and monitor the work of certified nursing assistants (CNAs) to ensure quality care and adherence to established protocols.

Requirements:

 

– Valid LPN license in the state of New York.

– Proven experience as an LPN, preferably in a long-term care or similar setting.

– Strong knowledge of nursing principles and practices, infection control protocols, and basic medical terminology.

– Excellent communication and interpersonal skills, with the ability to empathize and connect with residents, families, and fellow staff members.

– Ability to remain calm and composed in high-pressure situations, demonstrating critical thinking and problem-solving skills.

– Detail-oriented and highly organized, with the ability to multitask and prioritize responsibilities effectively.

– Flexibility in working shifts, including weekends and holidays, to provide 24/7 care to residents, as per the facility’s requirements.
