Key Responsibilities of the RN Hospice Case Manager

• Identify and address patient's and family's physical, psychosocial, environmental, safety, and developmental needs
• Individualize patient’s plan of care based on needs identified
• Document problems, nursing assessments, goals, and patient outcomes effectively
• Assess and manage all aspects of patient's pain with a personalized plan
• Prevent and treat undesirable symptoms or secondary symptoms
• Participate in interdisciplinary team meetings for comprehensive care planning
• Maintain accurate and up-to-date patient records
• Educate patient, family, and caregivers on interventions and medications
• Implement care to achieve outcomes of dignified and comfortable dying
• Support the patient's and family's spiritual and cultural beliefs
• Collaborate with the interdisciplinary team for consistent and quality care

Requirements of the RN Hospice Case Manager

• RN license
• Hospice experience
• Experience with HCHB (Homecare Homebase)
