Key Responsibilities of the RN – Home Health

• Assess, plan, implement, and evaluate patient care plans in consultation with healthcare professionals
• Provide direct and individualized bedside nursing care to pre-and post-surgery patients
• Administer and record prescribed medications
• Change dressings, insert catheters, and start IVs
• Adjust medical equipment and interpret measurements
• Monitor patients' vitals and report adverse reactions
• Collaborate on discharge planning with the healthcare team
• Educate patients and their families on surgical procedures and care
• Maintain and update patient files and medical records
• Adhere to best nursing practices, standards, and protocols

Requirements of the RN – Home Health

• Minimum two (2) years Med/Surg experience in acute care or post-acute facilities (most recent within the past year)
• Experience in home care and physical assessment
• Proficiency in medication administration, wound care, infection control, IV therapy, and post-operative care
• New York State RN License & Primary Source Verification
• AHA BLS and ACLS certifications (Queens area only)
• Associates Degree in Nursing from an accredited educational institute
