Location: Chicago, IL

Pay: $2,880.40 weekly

Shifts: Monday – Friday, 0800-1630 (Weekend rotation every 5th weekend)

Seven Healthcare is seeking an experienced RN Inpatient Care Manager specializing in case management to join a dynamic team in Chicago, IL. This travel assignment is focused on providing care to an underserved population, particularly adult and geriatric patients with complex cases. Candidates will work in a multidisciplinary environment and may float across various units including PCU, MedSurg, and LDRP.

Key Responsibilities of the RN – Case Management

• Manage a case load of 14-20 patients
• Collaborate effectively with multidisciplinary teams and physicians
• Navigate various insurance plans
• Assist with facility initiatives
• Utilize EPIC EMR and Careport for referrals

Requirements of the RN – Case Management

• Minimum 3 years of acute care case management experience
• ASN or higher educational qualification
• Strong assessment, critical thinking, and interpersonal skills
• Experience working with multidisciplinary teams
• Comfortable finding resources for patients
