Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for diagnosis, medical history, precautions, and contraindications
• Establish measurable goals and develop treatment plans for patient independence/function
• Re-assess treatment effectiveness and modify plans as needed
• Communicate patient needs and progress to treatment team and involved parties
• Consult with medical providers about evaluation, progress, and discharge plans
• Recommend adaptive equipment to enhance the rehab program
• Maintain and submit documentation according to regulations and standards
• Supervise Physical Therapist Assistants and Aides in patient care
• Participate in staff development and obtain continuing education

Requirements of the Physical Therapist

• Active NC PT license
• Experience in Home Health PT
• Proficiency in Oasis and HCHB
