Seven Healthcare are seeking a skilled Physical Therapist for a contract assignment in Pueblo, CO. This role requires an experienced physical therapist who is adept at treating patients based upon physician orders and referrals in an onsite setting. Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based upon physician orders/referrals.
• Complete comprehensive evaluations using appropriate tools and procedures.
• Review patient records for diagnosis, medical history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans to enhance patient independence and function.
• Regularly reassess treatment effectiveness and modify as needed.
• Communicate patient needs and progress with the treatment team and relevant parties.
• Consult with medical providers regarding evaluations, treatment progress, and discharge plans.
• Recommend and assist in securing adaptive equipment to improve patient function.
• Maintain and submit required documentation in compliance with regulations.
• Adhere to the Company’s standards, HIPAA, and Corporate Compliance programs.
• Supervise Physical Therapist Assistants and Therapy Aides per state regulations.
• Participate in staff development, meetings, and continuing education programs.

Requirements of the Physical Therapist

• PT doctorate license
• Fluent with point care
• Productivity requirement of 30 points a week
