Seven Healthcare are seeking a dedicated Physical Therapist for a full-time home health position in Orange County, Florida. This field-based role involves providing care across multiple agency partners, documenting within various EMR systems, and ensuring high-quality patient outcomes in a home health environment.

Key Responsibilities of the PT, Home Health

• Complete 30 visits per week, averaging 5-6 patient visits per day
• Conduct higher-level visits including SOC, Evaluations, Reassessments, and Discharges
• Coordinate daily visit schedules directly with patients
• Utilize Pariox EMR for schedule visibility and complete documentation within partner EMRs such as Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase

Requirements of the PT, Home Health

• Valid PT license in Florida
• Experience in home health settings
• Proficient in using various EMR systems
