Key Responsibilities of the Physical Therapist (Home Health)

• Conduct higher-level visits such as SOC, evaluations, reassessments, and discharges
• Document visits within various EMR systems including Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase
• Coordinate visit scheduling directly with patients
• Provide care across multiple home health agency environments
• Utilize central scheduling platform: Pariox EMR

Requirements of the Physical Therapist (Home Health)

• Valid Physical Therapy license in the state of Florida
• Experience in home health settings
• Proficiency in EMR systems
