Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Complete a comprehensive evaluation using appropriate tools and procedures.
• Review patient records for current diagnosis, medical history, and precautions.
• Establish measurable goals and develop a treatment plan to maximize patient function.
• Regularly reassess treatment effectiveness and patient progress.
• Communicate patient needs and progress with the treatment team and family.
• Consult with medical providers on patient evaluation and treatment.
• Recommend and assist in securing adaptive equipment to enhance rehab programs.
• Maintain documentation in compliance with required standards and regulations.
• Supervise Physical Therapist Assistants and Aides in patient care activities.
• Participate in staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist

• GA License
• Preferred: Experience with HCHB
• Licensed Physical Therapist with Home Health Care experience
