As a Physical Therapist, you will perform evaluations and treatments based on physician orders, with the support of a dynamic team and the flexibility to operate within a travel assignment framework.

Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
• Complete a comprehensive evaluation using appropriate tools and procedures
• Review patient records for diagnosis, past medical history, precautions, and contraindications
• Establish measurable goals and develop a treatment plan for patient independence and function
• Regularly reassess the treatment plan, patient progress, and need for modifications
• Communicate patient needs and progress to the team, physician, and relevant parties
• Consult with medical providers on patient evaluation, treatment, and discharge
• Recommend and assist in securing adaptive equipment for patient function
• Maintain and submit documentation in required formats and timelines
• Adhere to professional standards, company policies, and state/federal laws
• Supervise Physical Therapist Assistants and Aides in patient care activities
• Participate in staff development programs, meetings, and continuing education

Requirements of the Physical Therapist

• WV PT license or Compact license accepted
• Experience in Homecare, OASIS, HCHB, Exercise programming, and Rehab
