Key Responsibilities of the LPN

• Provide direct patient care as assigned
• Monitor patient status and report changes to the healthcare team
• Administer medications and treatments as prescribed
• Document patient care and progress in Point Click Care (PCC)
• Adhere to the minimum 2 hours cancellation notice policy

Requirements of the LPN

• Current LPN license
• BLS certification
• Experience in a post-acute care setting preferred
