Key Responsibilities of the LPN / LVN

• Provide patient care in a post-acute setting as required
• Complete charting in Point Click Care (PCC)
• Collaborate with nursing staff to ensure quality care
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes to appropriate personnel

Requirements of the LPN / LVN

• Valid LPN / LVN license
• BLS certification
• No BSN required
• Experience in post-acute care preferred
