Key Responsibilities of the LPN/LVN – Long Term Care – Travel

• Provide nursing care to residents with conditions such as CHF, dementia, DM, CKD, COPD, dysphagia, urinary retention, and wounds
• Participate in patient assessments and contribute to care planning
• Administer medications and treatments as prescribed
• Maintain accurate patient records using Point Click Care (PCC) charting system
• Coordinate with multidisciplinary teams to ensure comprehensive care

Requirements of the LPN/LVN – Long Term Care – Travel

• Valid LPN/LVN license
• BLS Certification
• Experience in long-term care settings preferred
